Initial screening tool for cognitive changes.  The 3-item brief standardized cognitive measure “Mini-Cog” is a well validated starting point that can be done at the annual wellness visit or for patients with cognitive complaints that takes < 5 minutes.  This resource includes validated lists of words that can be used for recall component as well as a second page with large circle for the clock draw.
