Geriatric Workforce Enhancement Program (GWEP)

Health Resources and Services Administration (HRSA) established GWEP centers to develop a healthcare workforce that maximizes patient and family engagement and improves health outcomes for older adults by integrating geriatrics with primary care. There are 44 GWEPs nationwide. 

The objectives of the Duke GWEP are to collaborate with community agencies, to recruit and train Geriatric Resource Teams (GRT’s) in primary care practices and provide resources for the GRT’s for performance improvement and clinical consultation in the care of older adults with complex needs. The Duke GWEP is also focused on engaging and empowering seniors and their family caregivers in learning about geriatric care management and resources, communication and advocacy and looks to expand expertise in geriatric care management through IP advanced traineeships

The Gero Practice Compendium is a collection of both clinical and community resources related to Dementia Care, Medication Management and Care Transitions pertaining to the care of vulnerable older adults residing in the community. You will find clinical resources such as toolkits, checklists, templates and patient education materials. In addition you will find links to connect you with referral suggestions and community-based resources for your  patients and their caregivers.  The community-based resources and referral suggestions are housed on the Durham Network of Care site. While the Gero Practice Compendium is designed to meet the needs of  healthcare providers in primary care settings, many of the resources are appropriate for any setting of care.  

This Compendium was built by leveraging the expertise and experience of the Duke Geriatric Workforce Enhancement Program (GWEP) faculty and community partners.  The development of the Gero Practice Compendium is supported by a Geriatric Workforce Enhancement Program (GWEP) grant (U1QHP28708) from the U.S. Bureau of Health Professions Health Resources and Services Administration (HRSA).
